LASSO Registration Checklist

To complete the registration process the following registration materials must be received by LASSO no later than July 15, 2006.
Checked items have been received by LASSO. Remaining registration materials must be provided to LASSO by July 15, 2006:
_____ LASSO Player Information and Medical Waiver
_____ 1” square photo (please put name and age on back)
_____ Parent Volunteer Form
_____ Copy of birth certificate for players new to GVSA
_____ GVSA Player Registration Form (signed by both parent and player)
_____ Club Fees (Checks payable to LASSO)
_____ Notarized MSYSA Medical Release Form (Notary Publics will be
available to try-outs or at most banks)
All items not completed at tryouts must be completed and
mailed to LASSO (address below)
by July 15, 2006.
